Liability Waiver
By signing below, I acknowledge that my child is participating in the Enrichment Collective’s (After-School Drama Club at Belmont Elementary School) I understand that participation involves normal physical activity and agree that The Enrichment Collective, its staff, and its partners are not responsible for any injury, accident, or loss that may occur during the program. I assume all risks associated with my child’s participation and release The Enrichment Collective from any liability.
Photo Release
I give my permission to Enrichment Collective LLC and it's agents and representatives to use photography and video of the above enrolled student(s) for use in publicity for this program or future programming by Enrichment Collective, LLC and your students schools, and PTA website and slides for meetings.
Assumption of the Risk
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including but not limited to, personal injury disability and death), illness, damage, loss, claim, liability or expense of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at sessions and programs presented by Enrichment Collective, LLC, session or participation in programming (claims). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge and hold harmless Enrichment Collective, LLC, it's employees, agents and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kid arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions or negligence of Enrichment Collective, LLC, it's employees, agents and representatives in any Enrichment Collective LLC programs.
By signing this, you are agreeing to all of the above conditions and waivers.
